The following things are REQUIRED for 6th Graders:
1. 6th Grade School Physical: (Must be completed less than one year prior to start of 6th grade.)
2. Tetanus/Diptheria/Pertussis(Tdap) booster vaccine: One dose is required for 6th graders ages 11 and older (regardless of when last dose was given). **For individuals who cannot receive pertussis-containing vaccines, Tetanus/Diphtheria (Td) vaccine may be given instead.**
3. Varicella (chickenpox) vaccine: Two doses, or proof of disease, is required for 6th graders ages 11 and older.
4. Meningococcal Conjugate (MCV) vaccine: One dose is required for 6th graders ages 11 and older.
